About CSS
The legal entity behind CSS is “CSS Kranken-Versicherung AG” (legal form: company limited by shares (AG), UID CHE-110.130.047), based in Luzern. The FOPH register does not assign the insurer to any insurance group. The Federal Office of Public Health (FOPH) lists the insurer under number 8 in its register of approved health insurers, as of 01.01.2026. The register gives its area of operation as “CH; EU, EFTA, UK”. In basic health insurance, CSS must accept every person there who is required to take out insurance, without a health questionnaire, and the benefits are the same at every insurer under the Health Insurance Act (KVG/LAMal).
In the 2024 financial year, an average of 1,537,730 people had basic health insurance with CSS. That is 17.1% of the 9,005,998 insured persons across all insurers. These figures come from the FOPH supervisory data (table T 5.01), which are published every year for all health insurers. Per insured person, the insurer took in an average of CHF 4,055 in premiums in 2024 and paid out CHF 3,955 in net benefits.
For 2026, CSS has approved premiums in 26 of 26 cantons. As the reference for the premium level, we use adults in the standard model with a CHF 300 deductible and accident cover. Across all of the insurer’s premium regions, this gives an index of 99.3, where 100 is the unweighted average of all insurers in the same regions. On average, the premiums are therefore 0.7% below the average. Compared with the respective cantonal average, the lowest index is in the canton of Appenzell Inner Rhodes (94.8) and the highest in the canton of Glarus (104.8).
Between 2011 and 2026, this reference premium at CSS changed, on average across its own premium regions, from CHF 370.71 to CHF 583.45 per month (+57.4%). Over the same period, the average of all insurers in the same regions went from CHF 343.85 to CHF 587.43 (+70.8%). The chart further down shows the trend year by year.
In 2026, CSS offers 3 model types: standard model (free choice of physicians), GP and HMO models and telemedicine and other models, with 5 tariffs in total, for example “Health practice insurance” and “Callmed”. Models with a restricted choice of service provider often apply only in certain regions; the premium calculator for your municipality shows whether a tariff is offered where you live.
In 2024, administrative costs came to CHF 156 per insured person (all insurers: CHF 193). The claims ratio, i.e. net benefits in relation to premiums, was 97.6% (all insurers: 94.9%). Reserves amounted to CHF 713.7 million, or CHF 464 per insured person. In the KVG solvency test as of 01.01.2025, CSS achieved a solvency ratio of 131%; the required minimum is 100%, while the figure for all insurers was 147%.
You cancel basic health insurance in writing. To switch on 1 January, your notice of cancellation must reach CSS by 30 November (Art. 7 para. 2 HIA); the FOPH recommends sending it by registered mail by 15 November. The insurer’s website does not give a separate cancellation address; we show the general postal address, which you’ll find further down. Why the solvency ratio matters
The solvency ratio shows whether an insurer’s reserves are sufficient to bear the risks of a bad year (KVG solvency test under the Health Insurance Supervision Act, KVAG). At 100%, the required minimum reserves are just in place; below that, the insurer must take action, for example by raising premiums.
In August 2025, the FOPH declared the insurer KLuG (Zug, around 9,300 insured persons) insolvent and withdrew its licence as of 31.12.2025. In the KVG solvency test as of 01.01.2025, its solvency ratio was 69%. The insolvency fund of the Common Institution under the Federal Health Insurance Act took over outstanding benefits; insured persons were able to switch or were taken over by Helsana.
